If you would like to use the questionnaire at your school, please follow these steps:
You will receive an e-mail with a web link where students can access the questionnaire.
The school should inform the students about the questionnaire at least two days before administering it to them, so the students can freely decide whether or not to participate.
The questionnaire is filled out at the computer classroom (see minimum requirements), where the students are given the web link to the questionnaire.
When students type in the web link, they will find on-screen instructions on how to complete the questionnaire, a notice that participation is voluntary, and a reminder that they can leave the room at any time if they wish.
While students answer the questionnaire, teachers should refrain from walking among students to ensure confidentiality of the responses.
When a student has finished, he/she can leave the classroom.
After students complete the questionnaire, the school confirms its completion with an e-mail to the following address: firstname.lastname@example.org
Within a few weeks the school receives a detailed and tailored report on the questionnaires’ overall results.
